Paperwork deadlines are when families submit all required documents, including medical clearance, report cards, and birth certificates for new players, to ensure athletes are eligible to participate. All required paperwork must be turned in before players can receive equipment or begin conditioning and practices.

Flex Paperwork Drop Off Location: 192 Gates Pond Road, Berlin
Tackle Paperwork Drop Off Location: 3 Gately Ave, Hudson

Required Paperwork:
• AYF Player Packet (must be printed and completed — instructions were sent in a separate HYFC email)
• Final report card (not required for athletes entering Kindergarten)
• Original raised-seal birth certificate (NEW athletes only — will be returned at Jamboree)
• Medical clearance (only if your athlete has not had a 2026 physical)

If your athlete already had a physical in 2026, please print their health form from their patient portal. It must:
• Be dated in 2026
• Be signed by a medical provider
• State they are cleared for all sports

Important:
All paperwork must be turned in and accounts must be up to date before athletes can begin practice.
