DESCRIPTION:Paperwork Deadline\n\nPaperwork deadlines are when families submit all required documents\, including medical clearance\, report cards\, and birth certificates for new players\, to ensure athletes are eligible to participate. All required paperwork must be turned in before players can receive equipment or begin conditioning and practices. \nFlex Paperwork Drop Off Location: 192 Gates Pond Road\, Berlin\nTackle Paperwork Drop Off Location: 3 Gately Ave\, Hudson \nRequired Paperwork:\n• AYF Player Packet (must be printed and completed — instructions were sent in a separate HYFC email)\n• Final report card (not required for athletes entering Kindergarten)\n• Original raised-seal birth certificate (NEW athletes only — will be returned at Jamboree)\n• Medical clearance (only if your athlete has not had a 2026 physical) \nIf your athlete already had a physical in 2026\, please print their health form from their patient portal.
